Developmental Disabilities Health Partners Corp. (DDHP) is a New Jersey non-profit corporation providing community based intensive in-home behavioral services for children and adolescents with intellectual and developmental disabilities (IDD). These services are uniquely tailored to the individual needs of children and their families. DDHP currently provides these services as part of the Children's System of Care (CSOC) of the New Jersey Department of Children and Families (DCF). PerformCare is the Administrative Service Organization (ASO) for New Jersey's Children's System of Care (CSOC). PerformCare personnel assist parents and caregivers with accessing CSOC services anywhere in New Jersey. PerformCare has trained personnel and integrated technologies to register, authorize, and coordinate services for children, youth, and young adults who are experiencing emotional and behavioral challenges, who have substance abuse problems, or children who have an intellectual/developmental disability. PerformCare is able to form a unique partnership among parents, family support organizations, service providers, and care managers to provide a central support group for children and youth in need. Parents and caregivers are integral to the process and serve as the primary force behind the planning and treatment process.
DDHP is one of the agencies contracted to provide in-home behavioral services once the child is registered and authorized by PerformCare.
